ELECTIONS REMINDER- ABSENTEE BALLOT REQUEST FORM
ELECTIONS REMINDER: To vote by absentee ballot you must fill out a new absentee ballot request form each year (unless you are an indefinitely confined voter as defined by the Wisconsin Statutes).
If you intend to vote by absentee ballot in the April, August or November 2022 elections, you must complete this form and return it to the clerk’s office by the deadlines preceding election day. You can get the form at the clerk’s office, or fill it out online at https://myvote.wi.gov/en-us/
